Nominations from the public are accepted every year.  The deadline to receive a nomination package is January 15.  Please read the instructions below before preparing your nomination package.

Due to COVID-19, we will be inducting our 2020 inductees at our 2021 celebration.  With that in mind, we are currently accepting nominations for 2022.

The form (which appears as the last page in the package) MUST appear as page one of your package.  It contains important contact information that we will need throughout the evaluation process.  All information will be kept private and confidential.
